What is the best way to model sleepers with subfloor over an existing concrete slab?

I would draw a floor system as your sleepers + subfloor. In floor mode while on your foundation drawing page, you can draw a floor system in place and set the height. Make sure to set your sub-floor,joists,rimjoist etc. to level 2 or something and de-select cleanup on each of them as well.
